Self-pay fees are due according to the practice's payment policy. Additional charges may apply for certain administrative services, forms, letters, or extensive record review when agreed upon in advance.
Cancellations & missed appointments
Your appointment time is reserved for you. Advance notice lets that time be offered to another patient who may be waiting for care.
Please provide at least 48 hours' notice to cancel or reschedule. Appointments canceled with less than 48 hours' notice, and missed appointments without notice, may be subject to a $150 late cancellation or no-show fee. Because insurance plans don't cover missed-appointment fees, this fee is the patient's responsibility and may be charged to the payment method on file through the secure billing platform.
Emergencies happen. Cancellation fees may be waived at the practice's discretion in cases of illness, family emergencies, severe weather, or other extenuating circumstances.
Repeated late cancellations or missed appointments may affect your ability to continue scheduling, since consistent attendance is an important part of effective treatment.
Administrative services
Forms, letters, and administrative documentation require clinical appropriateness, adequate evaluation, and sufficient treatment history when applicable. Additional policies or fees may apply, and completion isn't guaranteed.
